1995年、桐生一馬、錦山彰（錦）、澤村由美、錦山の妹・ミホの四人は、幼い頃に両親を失い、施設長の風間新太郎が運営する児童養護施設「ひまわり」で暮らしていた。
規律に縛られた暮らしから新しい世界へ飛び出そうと、桐生は、錦、由美、ミホと計画し、ゲームセンター襲撃事件を起こす。襲った店は神室町を牛耳るヤクザ組織、堂島組の息がかかっていたため、堂島組長から事件の落とし前をつけるよう迫られる。
桐生は地下格闘に出場し、由美とミホもまた、堂島支配下のクラブセレナでホステスとして働かされることになる。錦も小指を詰めようとしたことが堂島に認められ、桐生と錦は堂島組に組入りする。
元堂島組の若頭で、ヤクザの悪辣さを知り尽くしていた風間は、桐生たちがまっとうな人生を歩むことを望んでいたが、その願いとは裏腹に四人は裏社会に落ちていく…。
十年後の2005年、東城会で頭角を現した錦は堂島組長に、由美はニューセレナのママに収まっており、完成したミレニアムタワー内で意のままに権勢を振るっていた。
その矢先、由美の姉アイコが、愛人の大石とともに、近江連合の裏金百億を強奪するという事件を起こす。
会長の郷田を頂点とし、関西を拠点とする日本最大のヤクザ組織である近江連合は、由美や錦の事件への関与を疑い、東城会と近江連合の衝突は避けられない情勢となる。
二大ヤクザ組織の抗争は何としてでも阻止したい警察上層部は、ヤクザ社会に潜入させようと桐生を特赦で出所させる。
当初はヤクザの世界に戻ることを拒んだ桐生であったが、由美や錦に危険が及んでいることが伝えられると、二人を守るため、再び神室町に戻ることを決意。
